REACH procedure in your language

• Arabic – العربية: ھل ما زلت ا قلق ؟

• Cantonese – 廣東話: 您仍然感到 擔憂嗎？

• Greek – Ελληνικά: Ανησυχείτε ακόμη;

• Mandarin – 普通话: 您仍然感到 担忧吗？

• Vietnamese – Tiếng Việt: Quý vị vẫn còn lo ngại?

REACH* out for patient safety

If there is a sudden change in your or a loved one’s medical condition, don’t sit in silence. Follow these steps:

1. Talk to your nurse or doctor, then if needed
2. Ask to talk to the nurse in charge.
3. If you are still concerned, you have the right to activate a ‘REACH’ call:
   • Pick up your bedside phone and call 2222 OR use your mobile phone and call 03 9929 8444,
   • Say you are making a REACH call
   • State patient’s name, ward and bed number
   • An emergency team will attend
