Glaucoma Community Collaborative Care with Community Optometry

The Glaucoma Community Collaborative Care program is a commitment between the hospital’s glaucoma service and certain community optometrists to care for patients in a collaborative care arrangement.

The Eye and Ear is expanding our glaucoma collaborative care program with the aim of providing increased access to care for our glaucoma patients. We hope to strengthen our relationships with community optometry and promote patient-centred multidisciplinary care.

Eligible patients with stable glaucoma will see an optometrist participating in this program for follow up at planned intervals between appointments with an ophthalmologist at the Eye and Ear, while they are waiting for review at the Eye and Ear, with clinical details documented in the Eye and Ear patient record and communication between the hospital and the optometrist. If there are clinical concerns, an earlier review appointment at the hospital will be arranged.
